Inclusion criteria

Inclusion criteria: -Patients who have been diagnosed with CKD -Patients who can access the website and complete the survey -Patients willing to participate in a patient advisory board to be held at a later date -Patients whose consent has been obtained from the patient on the website prior to the start of this study

Exclusion criteria

Exclusion criteria: -Patients on maintenance dialysis (hemodialysis, peritoneal dialysis) or scheduled for induction -Patients who have undergone or will undergo kidney transplantation
