Inclusion criteria

Inclusion criteria: Patients with stage 3 or higher chronic kidney disease (eGFR <60 mL/min/1.73 m2) caused by diabetes without hemodialysis

Exclusion criteria

Exclusion criteria: 1. Cases with malignancy upon enrollment of the trial 2. Cases with active infections upon enrollment of the trial 3. Patients who did not agree to participate in this clinical trial

MeasureTime frame
1.Renal event is defined as the doubling of serum creatinine level, the initiation of renal replacement therapy, and death due to end-stage renal dysfunction. 2.To examine the cumulative incidence ratio of renal event during 5 years observation period after multidisciplinary team care. 3 To identify the beneficial or risk factors related to the occurrence of renal event.
