Inclusion criteria: 1. Patients admitting to the intensive care unit at Tokyo Women's Medical University Hospital 2. Patients with incidents or accidents in intensive care unit 3. The incidents or accidents are recorded to the incident reporting system The patients who fulfill all the conditions are enrolled.
