Inclusion criteria

Inclusion criteria: patients with cancer suffering from pain are enrolled if they are non-responsive to another opioid therapy(>=60 mg OME/day).

Exclusion criteria

Exclusion criteria: severe cognitive disorders QT prolongation contraindication for methadone
