Inclusion criteria

Inclusion criteria: 1)Age 65 or older at the time of consent 2)Taking one or more antihypertensive drugs 3)Taking drugs (including non-antihypertensive drugs) 2 to 4 times a day 4)Medications are packaged in one-dose 5)Systolic blood pressure 130 mmHg or higher at home or hospital 6)Poor medication adherence (correspond to either or both of (1) or (2)) (1)Medication Possession Ratio (MPR)*1 in the last two months is less than 90% or 110% or more (2)One or more of the drug management issues*2

Exclusion criteria

Exclusion criteria: 1)Pharmacist give home-care 2)Principal investigator may consider that assistance is required to take medicine 3)Residing in a place where the principal investigator cannot go within 2 hours 4)Principal investigator may consider unsuitable for participants
