Inclusion criteria: Person who taking more than 5 medications Person who agree to participate in the research

Exclusion criteria

Exclusion criteria: Cancer (palliative care) patients Psychiatric disorders Patients who have already reviewed the prescription Patients judged inappropriate by the facility manager
