Inclusion criteria

Inclusion criteria: Being treated for chronic disease Ability to read,provide written answer to questionnaires in Japanese No schedule for pregnancy, surgery, radiation therapy, chemotherapy Having informed consent

Exclusion criteria

Exclusion criteria: Being at hospital or institutionalized at baseline
