Inclusion criteria: Stroke patient Patient of modified Rankin Scale ; 0-3 Plan of discharged home

Exclusion criteria

Exclusion criteria: A severely handicappped Patient of modified Rankin Scale : 4-5 Patient of dementia Patient of terminal
