Using Discharge Planning tool, when acute or recovery hospital&#39
s physiotherapists and occupational therapist have conducted discharge Planning. Conducting usual discharge planning process by physical therapists and occupational therapists.

Inclusion criteria

Inclusion criteria: 1: Patients of acute hospitals or recovery hospitals and having physical therapy or occupational therapy. 2: Going to discharge for home and decided the date of prearrangement.

Exclusion criteria

Exclusion criteria: 1: Full independence of basic ADLs
