Conducted a physical therapy in our hospital admission, for a patient that ward within walking independence, to use the exercise notes.

Inclusion criteria

Inclusion criteria: 1: hospitalized patients to have performed physical therapy. 2: Walking independence in the ward 3: Exercise notes can be used

Exclusion criteria

Exclusion criteria: 1: Using the exercise notes is a difficult person 2: Who is difficult to fill in the questionnaire 3: those who risk of changes occur in significant vital signs and symptoms during the implementation is expected
