Heart failure or acute myocardial infarction with type 2 diabetes.

Inclusion criteria
Inclusion criteria: 1. Stable HF patients with NYHA II or above 2. Patients with AMI who have finished rehabilitation, and just before leaving hospital. 3. HbA1c level 6.5% (NGSP) or more, or diagnosed as diabetes, or taking antdiabetic medications. 4. Age more than 20 yrs.
Exclusion criteria
Exclusion criteria: 1. Patients with significant arrhythmia such as atrial fibrilation. 2. Patients with decompensated heart failure, fever, inflammation (CRP>1.0mg/dl) 3. Patients who are using insulin 4. Hepatic damage (AST, ALT >100 IU/L) 5. Patients with HBV, or HCV infection 6. Moderate renal dysfunction (serum creatinine >2.0mg/dl) 7. Patients with inflammatory bowel disease, colon ulcer, and bowel obstruction. 8. Severe chronic bowel diseases 9. Patients who have disease with increased bowel gas. 10. Patients with a history of gastric resection. 11. Lactose intolerance 12. Pregnant women or possible pregnancy. 13. Contraindication for miglitol 14. Dementia from whom we cannot obtain informed consent. 15. For whom the attending doctor considered he or she is not appropriate for the study.
