Inclusion criteria

Inclusion criteria: Participants are the beneficiaries of Kure-city national health insurance, aged over 40 years who received a specific medical check-up of 2012 and 2013, met criteria as described in the below, and provided informed consent to participate. Inclusion criteria were (1) urine protein level is higher than 2+, or (2) urine protein level is 1+ and an estimated glomerular filtration rate (eGFR) between 15 to 44 mL/min/1.73 m2 at the registration point, and (3) no diagnosis of diabetic nephropathy at the registration point.

Exclusion criteria

Exclusion criteria: Exclusion criteria were (1) having introduced RRT, (2) died, (3) cognitively impaired or mental disorders, and (4) when his/her primary physician judged that the enforcement of the program causes problems on the patients.

Design outcomes

Primary

MeasureTime frame
The number of subjects who moved to ESKD, initiated RRT, newly onset of CVD (cerebrovascular disease, coronary artery disease, and heart failure), and the number of death.

Secondary

MeasureTime frame
Physiological indicators: BP, Body Mass Index (BMI), urine protein, Scr, eGFR, BUN, UA, and non high-density lipoprotein cholesterol (non HDL-c), and other data. Psychological indicator: the self-efficacy scale on health behavior in patients with chronic disease. The process indicators: the proportion of days in a month that subjects performed the following self-management behaviors.
