Inclusion criteria

Inclusion criteria: Inpatients and outpatients at the age of 16 and upper with CKD including diabetic nephropathy

Exclusion criteria

Exclusion criteria: Patients with mental disorder, and patients receiving dialysis therapy or renal transplantation
