Inclusion criteria

Inclusion criteria: 1. Patients with CKD stage 1,2,4 or 5. 2. Patients with CKD stage 3 who have proteinuria, also with diabetes mellitus or hypertension

Exclusion criteria

Exclusion criteria: 1. Dialysis patients 2. Patients who undergoes treatment of CKD only at nephrologists, not at general practical physicians 3. Patients who undergoes treatment of CKD at physicians who do not register this study
