Inclusion criteria

Inclusion criteria: (1) Convalescent patients diagnosed with cerebral infarction and cerebral hemorrhage by CT or MRI examination for the first time; (2) Glasgow Coma Scale (GCS) score after cerebrovascular disease >8, the patient had no obvious disturbance of consciousness; (3) Those who are willing to participate in the study and sign the informed consent; (4) Return home after discharge, and the home address is within the main urban area; (5) Patients aged between 18 and 80 use smart phones by themselves or their escorts.

Exclusion criteria

Exclusion criteria: (1) Coma upon discharge; (2) patients with other critical diseases, such as malignant tumor, acquired immune deficiency syndrome, heart failure, renal failure, respiratory failure, liver failure, severe trauma, or cerebral cortex damage; (3) those who have a history of cognitive impairment or mental disorders; (4) Domicile transfer is large, and contact information is not fixed; (5) Does not meet the above diagnostic criteria.
