Control Group:The patient will receive regular health education on a hyperphosphatemia diet, conducted by healthcare professionals in outpatient clinics or wards through one-on-one health counseling.
Intervention Group:The intervention group implemented stage-based health education, including the unfreezing, change, and refreezing stages.

Inclusion criteria
Inclusion criteria: (1) Age = 18 years, on regular hemodialysis for at least 3 months; (2) Serum phosphorus level = 1.78 mmol/L (or 5.5 mg/dL) in the past 3 months; (3) Normal cognitive function, willing to participate in the study; (4) Educational level above primary school.
Exclusion criteria
Exclusion criteria: (1) Severe cognitive impairment or mental illness; (2) Coexisting malignant tumors; (3) Planned hospital transfer or kidney transplantation within 3 months.
