Interventions

Irrigation group:Routine treatment + bronchoscopic lung lavage with ambroxol. Routine treatment includes anti-infection, anti-inflammatory, fever reduction, and mechanical ventilation for respiratory
Yinqiaosan Formula:Conventional treatment combined with modified Yin Qiao San therapy

Inclusion criteria

Inclusion criteria: 1) Diagnosed with severe pneumonia (SP) according to the 'Chinese Expert Consensus on Clinical Practice of Severe Pneumonia in Emergency Departments'; (2) Presence of respiratory failure (RF); (3) Age = 60 years, regardless of gender; (4) Meeting the diagnostic criteria for phlegm-heat blocking the lungs syndrome as specified in the 'Guidelines for Traditional Chinese Medicine Diagnosis and Treatment of Community-Acquired Pneumonia' and the 'Diagnostic and Therapeutic Efficacy Standards for TCM Diseases': thick yellow sputum, severe cough with chest pain, high fever with restlessness, chest fullness and shortness of breath; poor spirit, chest pain with dry mouth, yellow urine and constipation; tongue and pulse manifestations include red tongue, yellow or greasy yellow coating, rapid or slippery rapid pulse; (5) Disease course = 72 hours; (6) The patient or their legal representative has signed an informed consent form.

Exclusion criteria

Exclusion criteria: 1) There are contraindications for bronchoscopy such as active massive hemoptysis, severe arrhythmia, aortic aneurysm, severe pulmonary hypertension, etc.; (2) Respiratory failure caused by non-infectious lung diseases such as acute pulmonary embolism and cardiogenic pulmonary edema; (3) Accompanied by other lung diseases such as lung cancer and pulmonary tuberculosis; (4) Accompanied by severe dementia or mental illness; (5) Accompanied by other serious physical diseases such as severe liver and kidney dysfunction, cardiovascular and cerebrovascular diseases; (6) Allergic constitution
