Interventions

Control group:Endotracheal intubation with invasive mechanical ventilation, routine symptomatic treatment
Treatment group:Endotracheal intubation with invasive mechanical ventilation, supplemented with modified Bu Zhong Yi Qi Tang formula in addition to routine symptomatic treatment.

Inclusion criteria

Inclusion criteria: Diagnostic Criteria for Respiratory Failure: Confirmed diagnosis of "respiratory failure" based on arterial blood gas analysis. Criteria for Difficulty in Weaning: 1) Failure of SBT; 2) Death within 48 hours post-extubation 3) Use of NIV within 48 hours post-extubation; 4) Reintubation within 48 hours post-extubation. Age =18 years; Meets the above diagnostic criteria; Meets the weaning failure criteria following initial SBT failure; Intubated and connected to invasive mechanical ventilation via a ventilator; Duration of mechanical ventilation >48 hours; No occurrence of deep coma; Family voluntarily signs informed consent.

Exclusion criteria

Exclusion criteria: Patients with difficult extubation due to neurological or muscular disorders; those with severe hepatic or renal insufficiency, irreversible brain death, or other conditions; patients with multiple organ dysfunction; those in severe coma or shock; individuals with allergic constitutions; those with unstable vital signs during observation or who die within 48 hours post-extubation; or those experiencing other severe complications.
