Resident-to-resident aggression and associated physical and psychological harms among older adults in residential care facilities in Hong Kong Mental and Behavioural Disorders

Interventions
Wait-list cluster randomised controlled trial in residential care facility units with a target sample of 174 PCWs and 370 residents. We will randomize resident care facilities 1:1 to the intervention
PCW and resident/proxy surveys at baseline, post-intervention, and 3-month follow-up
PCWs maintain ongoing records of RRA incidents over the follow-up period.

Inclusion criteria
Inclusion criteria: 1. Full-time personal care workers (PCWs) delivering direct care to older residents in participating residential care facility (RCF) units 2. Older residents in sampled units
Exclusion criteria
Exclusion criteria: PCWs: 1. Not in direct care roles 2. Part-time/temporary/agency staff 3. Not attached to participating units Residents: 1. Cannot provide consent and have no appropriate proxy 2. Severe medical instability or terminal illness precluding participation

PCWs’ detection and reporting of RRA incidents (number and characteristics of reported events over follow-up) measured using Short-Form Resident-to-Resident Elder Mistreatment Scale (R-REM), Staff version and the RRA Behaviour Recognition and Documentation Sheet (BRDS) at baseline (T0), 1 month post-baseline (T1) and 3 months post-baseline (T2) 2. Staff health and care competence assessed at baseline (T0), 1 month after baseline (T1), and 3 months after baseline (T2). The measures include: 2.1. Mental health measured by the Patient Health Questionnaire-4 (PHQ-4) 2.2. Well-being measured by the World Health Organisation - Five Well-being Index (WHO-5) 2.3. Burnout measured by the Maslach Burnout Inventory 2.4. Fatigue measured by the Occupational Fatigue Exhaustion Recovery (OFER15) scale 2.5. Residents’ exposure to RRA measured using the Short-Form Resident-to-Resident Elder Mistreatment Scale (R-REM), Resident version at baseline (T0), 1 month post-baseline (T1) and 3 months post-baseline (T2) 2. Resident physical outcomes (occurrence of injuries, accidents, falls) measured using standardized questions assessing the resident's experience of falls, accidents, or injuries at baseline (T0), 1 month post-baseline (T1) and 3 months post-baseline (T2) 3.
