Inclusion criteria

Inclusion criteria: 1. Prescribed 10+ repeat medications 2. Aged 18 years or over

Exclusion criteria

Exclusion criteria: 1. Undergoing terminal illness care 2. Too frail to enter into an 8-month study 3. Currently pregnant 4. Does not have the cognitive capacity to take part in the study, e.g. cannot provide informed consent 5. Does not have the emotional capacity to take part in the study, e.g. dealing with a family crisis
