Inclusion criteria

Inclusion criteria: Participants will be adults aged 40-74 who are eligible to be invited for a NHS Health Check and who are able to give consent for participation in the trial.

Exclusion criteria

Exclusion criteria: The GP practice will exclude patients they consider unsuitable for the trial (i.e., any patient where receiving an invitation will cause unnecessary stress due to mental health issues or a diagnosis of terminal illness for example)
