Inclusion criteria
Inclusion criteria: Children born at Wythenshawe Hospital, Manchester and Stepping Hill Hospital, Stockport between January 1996 and April 1998
Exclusion criteria
Exclusion criteria: Premature birth <32 weeks gestation
