Session 1 - initial interview, general assessment and baseline assessments.

Inclusion criteria
Inclusion criteria: All patients will be seen on their second or third clinic appointment.
Exclusion criteria
Exclusion criteria: Living outside the hospital catchment area or refusal to consent.
