Inclusion criteria
Inclusion criteria: 1. Adult age with a primary diagnosis of schizophrenia, bipolar disorder, or major depression 2. Have been identified as high utilisers of psychiatric inpatient services
Exclusion criteria
Exclusion criteria: Primary diagnosis of substance abuse
