Inclusion criteria

Inclusion criteria: All patients (casualty and direct from general practitioner) admitted to the participating departments of elderly care with a postcode address corresponding to the locality served by the seven Community Hospitals will be eligible for inclusion to the trial. The patient must be sufficiently medically stable for the non-district general hospital care to be clinically appropriate.

Exclusion criteria

Exclusion criteria: Acutely ill or medically unstable patients will be exluded.
