Inclusion criteria
Inclusion criteria: Nursing homes are eligible if: 1. There is active engagement from: nursing home management, coordinating and advisory physician, head nurse and palliative support team (if present), meaning: 1.1. The nursing home management, coordinating and advisory physician, head nurse and palliative support team (if present) agree that participation in the study is a priority for the nursing home 1.2. The nursing home management agrees to provide time for the staff to participate in activities related to the implementation of the Care Programme for the Last Days of Life such as information sessions, training workshops, brainstorm sessions, feedback moments, coaching activities, reflective debriefings etc. 1.3. The nursing home management agrees to appoint three staff members, of which at least one from hands-on staff (e.g. nurse or care assistant) as local project champions. These staff members are dedicated to work in close cooperation with the researcher and will facilitate the PDCA cycles in the nursing home. These three persons will lead the project. The management will allow these persons to execute their roles as good as possible and will support them 2. The number of beds in the nursing home is at least 80
Exclusion criteria
Exclusion criteria: Nursing homes are not eligible if: 1. The nursing home is already participating in another project or change trajectory focussed on improving end-of-life care 2. A large re-organisation or change in staff is taking place or is planned during the study period
