Inclusion criteria
Inclusion criteria: Patients receiving polypharmacy defined as five or more chronic medications who were found to be non-compliant upon first assessment by pharmacist
Exclusion criteria
Exclusion criteria: 1. Speak non-Cantonese dialects or a different language 2. Conditions that prevented effective communication (for example, patients who are deaf, mute, or have dementia or other psychological disorders) 3. Live in nursing homes with supervised treatment
