Inclusion criteria

Inclusion criteria: Eligible to be invited for a NHS Health Check. NHS Health Checks are offered to all individuals aged 40 to 74 years old and who do not already have heart disease, kidney disease, diabetes or have previously had a stroke.

Exclusion criteria

Exclusion criteria: Ineligible for a NHS Health Check
