Inclusion criteria
Inclusion criteria: 1. Prescribed non insulin medication for type 2 diabetes for at least 2 years 2. Willing to give consent 3. Registered with one of the 4 participating GP practices
Exclusion criteria
Exclusion criteria: 1. Deemed unsuitable for inclusion in the study for any reason by their GP 2. Enrolled in other clinical trials 3. Suffering from terminal illness
