Inclusion criteria

Inclusion criteria: 1. Residential/care home managers 2. Residents of residential/care homes in Stockton-on-Tees who are aged 65 years or over, either sex 3. Health care professionals, including a representative sample of GPs, HF nurses, and care home staff, involved in care provision will be invited to participate in interviews

Exclusion criteria: Residents with terminal disease
