Approximately 5,000 patients presenting with heart failure to 9 hospitals including teaching, regional and municipal hospitals will be recruited and evaluate

Inclusion criteria
Inclusion criteria: 1. Patients with confirmed heart failure 2. Patients 13 years or older 3. Willingness to participate in the study
Exclusion criteria
Exclusion criteria: Participants who are unwilling to participate in the study
