Inclusion criteria
Inclusion criteria: 1. Aged 65 years or over 2. Recently discharged from hospital 3. Capable of understanding and answering questions in French
Exclusion criteria
Exclusion criteria: 1. Outpatient treatment still going on within the hospital premises 2. A medical prescription for a single intervention of home health care 3. If they were outside the study reach
