Interventions

Diode laser-assisted tooth extraction using a 970 nm diode laser (IRE-Polus, Russia). Laser settings: wavelength 0.97 µm

output power 2.4 W for periodontal ligament dissection, 1.5 W for bactericidal treatment of the socket

pulsed mode (pulse duration 600 ms, pause 400 ms)

exposure 8–12 s for circular ligament separation, 6–30 s for socket curettage, 3–6 s for bactericidal treatment. The laser is used for periodontal ligament dissection, socket curettage and bactericida

Inclusion criteria

Inclusion criteria: 1. Age =18 years 2. Chronic apical periodontitis (ICD-10: K04.5) 3. Clinical indication planned for tooth or root extraction 4. Ongoing antiplatelet therapy (continuous antiplatelet therapy for =3 months with aspirin (75-150 mg/day), clopidogrel (75 mg/day), or dual antiplatelet therapy (DAPT: aspirin+clopidogrel))

Exclusion criteria

Exclusion criteria: Exclusion criteria (baseline) : 1. Age <18 years 2.Acute infectious processes requiring emergency extraction 3. Decompensated systemic disease 4. Oral anticoagulant therapy (warfarin, direct oral anticoagulants) 5. Known coagulopathies (hemophilia, von Willebrand disease) 6. Immunosuppressive therapy or chemotherapy 7. Pregnancy or lactation 8. Local contraindications (suspected malignancy, acute osteomyelitis) 9. Active psychiatric disorders 10. Patient refusal of the laser procedure 11. Inability to complete follow-up protocol Exclusion criteria (during study): 1. Acute exacerbation of systemic disease 2. Intercurrent infectious illness 3. Non-adherence to medical instructions 4. Withdrawal of informed consent
