Inclusion criteria: 1. Adults aged 40–74 years 2. Without pre-existing diabetes or CVD 3. Has not attended an NHS Health Check in the last 5 years

Exclusion criteria: 1. Adults younger than 40 and older than 74 years 2. Has pre-existing diabetes or CVD 3. Attended NHS Health Check in the last 5 years
