Intervention group: Eligible patients will receive oral pioglitazone in addition to the standard treatment for nephrotic syndrome. Pioglitazone will be administered at a dose of 0.5 milligrams per kil

Inclusion criteria

Inclusion criteria: Age between 2 and 18 years Diagnosis of frequently relapsing Or diagnosis of steroid-resistant nephrotic syndrome Receiving prednisolone at a minimum dose of 0.5 mg/kg/day Estimated glomerular filtration rate (eGFR) > 60 ml/min/1.73 m² Written informed consent obtained from parents or legal guardians

Exclusion criteria

Exclusion criteria: Diabetes or abnormal blood glucose eGFR = 60 mL/min/1.73 m² Underlying cardiac disease Heart failure Secondary nephrotic syndrome

Primary

MeasureTime frame
Reduction in urine protein-to-creatinine ratio (UPCR) after pioglitazone administration compared with baseline in pediatric patients with complicated nephrotic syndrome. Timepoint: Measurement of urine protein-to-creatinine ratio (UPCR) at baseline (before initiation of pioglitazone treatment) and 3 months after the intervention. Method of measurement: Measurement of urine protein-to-creatinine ratio using urine sample analysis and standard biochemical laboratory methods.

MeasureTime frame
Change in serum albumin level after administration of pioglitazone compared with baseline in pediatric patients with complicated nephrotic syndrome. Timepoint: Measurement of serum albumin level at baseline (before initiation of pioglitazone treatment) and three months after the intervention. Method of measurement: Measurement of serum albumin level using blood sample analysis and standard biochemical laboratory methods.
