Inclusion criteria: "PICU admission" " Mechanical ventilation for more than 48 h"

Exclusion criteria

Exclusion criteria: "Primary or secondary immune deficiency" "Acute severe pancreatitis" "Significant gastro intestinal tract injuries(ileus, bleeding,...) "No Probiotics in the past 2 weeks" "Increased risk of endovascular infections (artificial heart valve, intravenous graft, Heart trauma, history of rheumatic fever)" "Tracheostomy" "Expected extubation less than 48 hours"

Primary

MeasureTime frame
Incidence of ventilator associated pneumonia (VAP). Timepoint: Check for pneumonia at the time of intubation and then daily until extubation. Method of measurement: According to ACCP(American College Of Chest Physician) the diagnosis of VAP is given a follow: The appearance of a new or progressive infiltrate on CXR along with 2 or 3 of the following : body temperature above 38.5 or bellow 35, leukocytosis above 10000 or leucopenia bellow 3000, purulent sputum. of increased pulmonary secretion.

"Duration of Mechanical ventilation" , "Duration of stay in PICU" , "Duration of Antibiotic use". Timepoint: "Time of intubation and extubation" , "First day of PICU admission and day of discharge" , "Antibiotic start date for VAP and when to stop them". Method of measurement: Counting the days.
