Inclusion criteria

Inclusion criteria: Adults aged 18–40 years Vertical and anteroposterior midface deficiency requiring Le Fort I osteotomy Planned maxillary repositioning = 6 mm without need for bone graft Candidate for VSP-assisted surgery Ability to undergo CBCT imaging ASA I-II Health status Ability to provide written informed consent

Exclusion criteria

Exclusion criteria: Need for maxillary advancement > 6 mm or requirement for bone graft Syndromic craniofacial anomalies History of cleft lip and/or palate Previous orthognathic surgery Systemic disease classified as ASA III or higher Pregnancy or breast feeding Inability to obtain required CBCT imaging

Design outcomes

Primary

MeasureTime frame
Accuracy of maxillary repositioning measured as the linear difference between the preoperative virtual surgical planning plan and the postoperative skeletal position in anterior and vertical dimensions. Timepoint: Three months after surgery. Method of measurement: Linear measurements in millimeters comparing the preoperative virtual surgical planning model with the postoperative cone beam computed tomography image in anterior and vertical dimensions.

Secondary

MeasureTime frame
Linear measurement of bone contact at maxillary buttresses after Le Fort I osteotomy, recorded in millimeters on postoperative CBCT. Timepoint: Three months after surgery. Method of measurement: Linear bone contact is measured on postoperative cone-beam computed tomography using a standardized imaging protocol.
