Inclusion criteria

Inclusion criteria: Age 65 years and above Concurrent use of 5 or more medications Ability to communicate (verbally or written) Achieving a quorum score on the AMT questionnaire Having a mobile phone or access to a telephone line Informed consent to participate in the study Having a caregiver is mandatory if the person has cognitive and physical impairments

Exclusion criteria

Exclusion criteria: Lack of communication for more than a week Inability to follow up on ongoing calls Severe cognitive impairment Hospitalization or acute clinical change Death

Pharmaceutical literacy. Pharmaceutical management. Timepoint: Beginning of the intervention and end of the intervention. Method of measurement: Questionnaire : MEDMAIDE . SELF-MED . MHLM.
