Inclusion criteria

Inclusion criteria: Age 0 to 18 years History of head trauma of any severity and mechanism Decision of the attending physician to perform a CT scan based on clinical protocols Obtaining written informed consent from the parent or legal guardian

Exclusion criteria

Exclusion criteria: Eye injuries that interfere with measurement History of eye or optic nerve surgery Neurological or metabolic diseases affecting ICP or ONSD Severe orbital hematomas or fractures that interfere with ultrasound Inability to perform CT scan

MeasureTime frame
Optic nerve sheath diameter (ONSD). Timepoint: First visit to the emergency room. Method of measurement: Using ocular ultrasonography The patient is placed in a supine position, and their eyes are kept in a straight-ahead gaze position with closed eyelids. After ensuring the child is calm, a sufficient amount of ultrasound gel is applied to the closed eyelid of each eye.A high-frequency linear probe (in the range of 7.5 to 13 MHz) is then used. The probe is gently placed without applying direct pressure on the eyeball to obtain a suitable longitudinal view of the optic nerve and the optic disc.In the acquired image, the Optic Nerve Sheath Diameter (ONSD) is measured at a distance of 3 mm posterior to the globe, from the point where the optic nerve connects. This measurement is performed by determining the distance between the two reflective, bright (echogenic) lines that mark the inner boundaries of the optic nerve sheath.The measurement is performed separately for each eye, and the average of the two values is recorded as the final Optic Nerve Sheath Diameter.The obtained values are compared to normal ranges based on age. Typically, a diameter of less than 4.5 to 5 mm in children is considered normal, and values higher than this limit may indicate Increased Intracranial Pressure (ICP).
