Inclusion criteria

Inclusion criteria: Having an endotracheal tube or tracheostomy RDOS scale above 3 Active expiratory breathing pattern Reducing tidal volume High PIP airway pressure and high Pplat pressure depending on the type of ventilator mode (pressure and volume) Flow-time and flow-volume loops indicate airway obstruction.

Exclusion criteria

Exclusion criteria: Removal of the endotracheal tube before the end of the study 2.Patient death

Timepoint: The number of suctions is not fixed and suction is based on the patient's needs and the RDOS criterion is measured by the researcher 3 minutes after each suction and will continue until the RDOS criterion is <3 depending on the respiratory distress status. In this study, the amount of changes in graphs and loops, changes in PIP and Pplat according to the mode used, vital signs (pulse, systolic and diastolic blood pressure and respiratory rate) will be examined and recorded before and after suction. ABG control will be examined to determine the effects of bicarbonate absorption from the trachea and its effect on blood pH. Method of measurement: Sodium bicarbonate, how to measure group A / N-acetylcysteine, how to measure group B / Excretion, how to measure Respiratory Distress Observation Scale (RDOS) / Pplat changes, measuring unit in centimeters of water / blood oxygen saturation (SpO2), measuring unit in percent / End tidal co2 (Etco2), measuring unit in millimeters of mercury / Ppeak changes, measuring unit in centimeters of water / Flow-time curves (expiratory) / Flow-time curves, measuring unit with or without obstruction / Flow-volume loops, measuring unit with or without ob
