Inclusion criteria

Inclusion criteria: Resident of Iran and fluent in Persian with a primary diagnosis of OCD based on DSM-5 criteria and a score greater than 16 on the Yale-Brown Obsessive-Compulsive Disorder Scale. Participants taking psychiatric medications are eligible if they have been on a stable dose for at least two months and agree to maintain this stable dose throughout the study. Participants with comorbid depression or anxiety disorders are eligible only if their OCD is the most severe and disabling condition. Participants are required to have regular access to a computer to access the Internet.

Exclusion criteria

Exclusion criteria: Diagnosis of bipolar disorder or psychosis. Current substance abuse or dependence; Clinically significant suicidal ideation or history of suicide attempt or hospitalization Hoarding as a primary symptom of OCD5 Severe OCD (score greater than 31 on the Yale-Brown Obsessive-Compulsive Disorder Scale) A current serious physical illness that could interfere with ICBT.

Changes in treatment outcomes and impact on quality of life and well-being. Timepoint: Pre-, post- treatment, four-month follow-up. Method of measurement: Revised Obsessive-Compulsive Disorder Inventory, Hamilton Depression Scale (HDRS), World Health Organization Quality of Life Short Form (WHOQOL-BREF), Negative Effects Questionnaire (NEQ), Clinical Treatment Implications Scale (CGI<2).

Decrease of OCD severity. Timepoint: Pre-, post- treatment, four-month follow-up. Method of measurement: Yale-Brown Obsessive-Compulsive Disorder Scale (Y-BOCS) (35% reduction in severity score).
