MeasureTime frame
Self-care score in the Tobert questionnaire and readmission rate. Timepoint: Measuring the level of self-care at the beginning of the study and 6 months after the end of the intervention and examining the rate of readmission within 6 months after the end of the intervention. Method of measurement: Tobert Self-Care Questionnaire.
