Inclusion criteria
Inclusion criteria: primary varicocele unilateral on the left side small testis, where the difference of 10% of the testis volume or more with the testis on the healthy side was considered as a small testis as a control due to infertility and decreased sperm count Semen analysis has been referred
Exclusion criteria
Exclusion criteria: simultaneous presence of hydrocele simultaneous presence of hernia sensitivity to papaverine

| Examining the preservation of the artery and its pulse using Doppler ultrasound. Timepoint: 3 months after intervention. Method of measurement: Doppler ultrasound device.;Examining the difference in the size of the testicles using Doppler ultrasound and the formula (length*width*height*52.) and physical examination. Timepoint: 3 months after intervention. Method of measurement: Doppler ultrasound device, physical examination.;The level of difficulty of the surgery will be evaluated based on the surgeon's evaluation scale regarding the difficulty in diagnosing and preserving the arteries with the scale of the difficulty of the surgery (difficult = 3, moderate = 2, easy = 1). Timepoint: During surgery. Method of measurement: Assigned numbers depending on the degree of difficulty.;Spermogram parameters will be compared using semen analysis before and three months after the operation.
