60 Years to 74 Years

Inclusion criteria

Inclusion criteria: Age 60-74 years Willingness to participate in a rehabilitation program Ability to use and access a smartphone Type 2 and 3 heart failure Reading, writing and speaking Farsi literacy Having a maximum of two single and permanent caregivers At least six months of caregiving experience for caregivers Failure to receive any training or participation in previous rehabilitation programs by older adults or their caregivers Absence of any cognitive and movement disorders

Exclusion criteria

Exclusion criteria: Elderly died during intervention Failure to continue cooperation by older adults Failure to complete more than one third of the questionnaire by older adults and their caregivers

Lifestyle changes. Timepoint: Completing the questionnaire before the intervention, one and two months after the end of the intervention. Method of measurement: The health-promoting lifestyle profile.
