Interventions

Intervention 1: Intervention group: the intervention group receives intravenous thrombolysis (IV TPA) with a dose of 0.9 mg/kg along with 20 mg of oral melatonin daily for three days (60 mg in total)

Inclusion criteria

Inclusion criteria: Patients with diagnosis of acute ischemic stroke and confirmed in CT scan or MRI according to NIHS (NIHSS 4-24) and recieving rTPA

Exclusion criteria

Exclusion criteria: CT SCAN findings of intracranial bleeding, iatrogenic stroke, vasculitis, vascular dissection, prior use of neuroprotective drugs such as edaravone, people with malignant tumors or receiving anticancer drugs and chemotherapy, people with severe underlying disease and life expectancy less than 90 days, pregnancy, breastfeeding, history of sensitivity to melatonin, MRS greater than 2 before stroke, NIHSS greater than 25 on admission, intubation on admission, NPO on admission, liver failure, renal failure, autoimmune disease such as rheumatoid arthritis , use of immunosuppressive drugs, seizures

NIHSS SCORE?MRS SCORE ?The mortality rate in the hospital, the rate of need for mechanical ventilation during the first two days, the rate of drug side effects, the length of hospitalization, the need for ICU, the rate of hemorrhagic transformation. Timepoint: Upon arrival, upon discharge and three months after discharge. Method of measurement: Based on NIHSS(National Institutes of Health Stroke Scale) SCORE?MRS(Modified Rankin Scale) SCORE.
