Inclusion criteria

Inclusion criteria: Adults aged 18 to 65 years Confirmed diagnosis of Chronic Kidney Disease by a specialist based on patient records Medical diagnosis of Chronic Kidney Disease for at least 3 months Ability to speak Persian Awareness of time, place, and person Proficiency in using online communication platforms Willingness to participate in the study ?Access to a smartphone Ability to work with a smartphone Literacy in reading and writing

Exclusion criteria

Exclusion criteria: Diagnosed psychological or cognitive disorders Visual or hearing impairments Participation in similar care programs Involving the patient or their family as part of the treatment team.

Self-Care. Timepoint: Self-care measurement at the beginning of the study (before the intervention began) and at the end of the 8th week of the intervention. Method of measurement: Wang Chronic Kidney Disease Self-Care Questionnaire 2019.;Medication Adherence. Timepoint: Medication Adherence measurement at the beginning of the study (before the intervention began) and at the end of the 8th week of the intervention. Method of measurement: Hill-Bone Medication Adherence Questionnaire 1999.
