Inclusion criteria

Inclusion criteria: Age above 18 years Not having problems such as mental and behavioral disorders and all kinds of retardation Not having physical problems such as hearing, vision and speech Lack of close or distant participation in similar educational projects Having the ability to read and write Having diabetes for at least six months Having a mobile phone as well as a landline at home Be a native of Semnan province Full satisfaction of the patient and her family to participate in this program Complete consent of the patient and her family for continuous telephone follow-up through the patient's cell phone or the landline of the residence Having type 2 diabetes A level equal to or greater than 6.5% of hemoglobin A1c

Exclusion criteria: The unwillingness of the patient to cooperate in this research plan death of the patient Absence of more than two sessions Dissatisfaction of the patient and her family with continuous remote follow-up Re-hospitalization of the patient Diabetic neuropathy disorder Having any other type of diabetes

MeasureTime frame
The score that the patient gets from the patient self-management questionnaire is 35 questions with a score of 175-35. Timepoint: It is before the start of the intervention and eight weeks after the end of the intervention. Method of measurement: Patients' self-management questionnaire is a standard tool to measure patients' self-management ability, which evaluates patients' self-management ability in 5 dimensions: self-organization, self-regulation, interaction with health professionals, self-monitoring, and adherence to diet. The dimension of self-regulation consists of 10 questions, which include questions about the ability to simultaneously advance the disease with daily life activities such as weight control and physical activities. The ability to monitor the symptoms of the disease is in line with diabetes. Each of the dimensions of interaction with health professionals and influential people, self-monitoring, as well as adherence to the diet are measured in the form of 9, 4, and 3 questions, respectively. The questions are scored on a five-point Likert scale from completely agree to completely disagree, respectively from 5 to 1. The score range is from 35-175, so that a higher score indicates a better situation in that specific dimension.;The score that the patient gets from the life satisfaction questionnaire is in the range of 35-5. Timepoint: It is before the start of the intervention and eight weeks after the end of the intervention. Method of measurement: Life satisfaction questionnaire is designed to measure the overall level of life satisfaction. This scale is actually a self-report tool that includes 5 propositions and the method of scoring each proposition is based on a 7-point Likert scale (from completely disagree to completely agree). The range of the scale scores is between 5-35, which obviously means that a higher score indicates greater satisfaction with life.
