Interventions

Intervention 1: Intervention group: fibrinilytic therapy with streptokinase 2500000 IU IV bolus then 10000 IU/hour IV infusion 12-24 hour. Then bridging therapy will initiate within two days after PT

Inclusion criteria

Inclusion criteria: Patients with high-risk Submassive pulmonary emboli;those patients with acute PE without systemic hypotension but with evidence of either right ventricle (RV) dysfunction or myocardial necrosis

Exclusion criteria

Exclusion criteria: Patients with history of chronic thromboembolic pulmonary hypertension (CTEPH)

Primary

MeasureTime frame
Prevalence of chronic thromboembolic PH(CTEPH) after 3 month therapy (anticoagulant or fibrinolytic) in two groups. Timepoint: comparision of mean LVEF after 3 month standard anticoagulant therapy. Method of measurement: comparision of mean RV function with TAPSE after 3 month standard anticoagulant therapy.
