Intervention 1: control group: paracetamol is pumped to the pump at 33 mg /kg/h for 24 hours and fentanyle 0.5 macro/kg/h and resuspended in normal saline to 100 cc. Intervention 2: intervention group

Inclusion criteria

Inclusion criteria: Patient with lower extremity fracture disese Patient with 18 -70 years old

Exclusion criteria

Exclusion criteria: History of the disease psychiatry History of the heart failure ,liver ,kidney ,lung consciousness abnormalities

Design outcomes

Primary

MeasureTime frame
Post operation pain after orthopedic surgery in lower limbs. Timepoint: Ricovery, 4, 8, 12, 24 hours after surgery. Method of measurement: VAS (visual analogue scale).

Secondary

MeasureTime frame
Sedation score. Timepoint: Ricovery, 4, 8, 12, 24 hours after surgery. Method of measurement: Questionare of ramsy score.
