Inclusion criteria

Inclusion criteria: Inclusion criteria: Type two diabetic patients male or female; age range 40 to 75 years; live in the city of Mashhad; provide informed consent. Exclusion criteria: Participating in another interventional study simultaneously; gestational diabetes; patients who refuse to participate in the study for any reason or their full cooperation may not be available.

MeasureTime frame
Self-awareness status. Timepoint: Baseline, immediately after end of intervention. Method of measurement: Interview about patients' awareness of their clinical condition.

Secondary

MeasureTime frame
Fasting blood sugar test at home once a week. Timepoint: One month after intervention, two months after intervention, immediately after end of intervention. Method of measurement: Note down in patient record.;Visit appointment adherence. Timepoint: One month after intervention, two months after intervention, immediately after end of intervention. Method of measurement: Note down in patient record.
